Youth Gardening Camp Waiver & Consent
I, the parent/guardian of the participating child, give permission for my child to attend and participate in all activities of the Love Alwayz Sproutz Youth Camp. I understand that while reasonable precautions will be taken to ensure safety, outdoor and gardening activities involve inherent risks, including but not limited to minor cuts, insect bites, allergic reactions, or weather-related exposure.
I release and hold harmless the camp organizers, staff, volunteers, and property owners from any liability or claims arising from participation, except in cases of gross negligence. I affirm that my child is physically able to participate and that I have disclosed any relevant medical conditions.
I also grant permission for my child’s photo to be taken during camp activities and used for promotional purposes, unless I notify the camp in writing.
By signing below, I acknowledge that I have read and understood this waiver and give my full consent.
